This presentation is not to point blame at anyone. The object of this presentation is to point out the stark reality of how easily a pilot can let their guard down and become a statistic of Human Error.

To promote a positive safety culture we need to mentor each other. The accident rate is so low in this country that to get to the next level, attitudes need to change concerning the guy that “everyone knew” would be the next casualty. That person needs to have a helping hand and open mind offered in the name of safety.
