Steve Lasday, FEDEX Feeder Safety System management (SMS) manager, and Greg Feith retired NTSB Accident Senior Lead Investigator, will take on a journey on how Human factors is the number one cause of failures in and out of the cockpit.
They will discuss:
1. This concept interchangeably termed Procedural, Operational, Practical, or Safety Drift
2. This briefing –will use Safety Drift as our descriptor
3. Definition -continuum between textbook compliance and how the procedure is being performed in the real world
4. Note the nomenclature “textbook” above, that context relates to any standardized and accepted procedural status (SOPs, policy, OEM requirements, regulations, etc.) At the 4thof July BBQ, remember those New Year’s resolutions??
5. Root causes of Safety Drift are many –Human Factors, organizational, training deficiencies, resource allocation, normalization of deviance
