Brief Description:

This safety seminar is an accident case study of a DHC-6 Twin Otter fuel exhaustion accident in Canada. A failure by the crew to request fuel prior to the accident flight, check the actual quantity of fuel onboard and adaptations made by the crew in the use of checklists led to an off-airport landing with minor injuries to the crew and passengers and substantial damage to the aircraft. This study will examine how we can prevent adaptations and normalization of deviance to ensure safer flight.
