Title:
Monthly meeting of Columbus EAA IMC Club Mastering the Art of Instrument Navigation
Topic:
Realigning the Aviation Tumblers to Occasion Safer Flights
Date and Time:
Wednesday, February 27, 2019, starting at 19:00 Eastern Standard Time Download Calendar File
Speaker(s):
Tim Heron
Brief Description:

The “Swiss Cheese” model of accident/incident causation illustrates that despite barriers, weaknesses and systemic flaws in those existing defenses (tumblers) may align under just the right conditions to set the occasion for an undesirable outcome. After viewing a scenario-based, animated video where holes in both ATC and pilot barriers aligned, an interactive discussion will follow on possible preflight, en route, and approach remedies to address future situations.
