This event will provide an overview of the Human Factors Analysis and Classification System (HFACS)based on research by Scott Shappell of the FAA Civil Aeromedical Institute in aircraft accident causation. We will address the various latent failures that lead to the active failures in 70-80% of aircraft accidents.
