Title:
An Advisory Only Message Leads to Tragedy
Topic:
Improper or Inadequate Pilot Decision Making Can Be Fatal
Date and Time:
Friday, March 27, 2026, starting at 12:00 Eastern Daylight Time Download Calendar File
Speaker(s):
Joey Keahey, FAASTeam Lead Representative
Brief Description:

Despite significant advancements in technology, pilot training, and newer, more advanced aircraft, approximately 80% of aviation accidents continue to be directly related to improper or inadequate pilot decision making. Failure to use checklists, using the incorrect checklist, pressure to make the flight, lack of knowledge, complacency, and loss of control are common human factors in accidents.

In this discussion, we will focus on human factors and how we can be better equipped to improve aviation safety.
